Other very big institutes with a lot of email users are universities, which are also switching to Gmail or Live Mail (and maybe other providers).

Also, if you really like Exchange (for some reason), there are services that provide hosted Exchange server -- you pay per mailbox, and don't have to handle the servers yourself.
